Affidavit of Domestic Partnership

The Affidavit of Domestic Partnership form and instructions are available under “Forms.” Please mail in your completed amendment form and fees to Saratoga County Sheriff’s Office (ATTN: Pistol Permits), 6012 County Farm Road, Ballston Spa, NY 12020. If there is an issue that you feel needs to be discussed in person please call us at (518) 885-2465. Our office is open to the public Tuesday through Friday 9:00 AM-4:30 PM.
