The amendment form and instructions are available under Forms. Please mail in your completed amendment form and fees to 6012 County Farm Road Ballston Spa, NY 12020. If there is an issue that you feel must be discussed in person please call us at (518) 885-2465. Our office is open to the public Tuesday-Friday 9-4:30pm.

*Amendment form must accompany the Duplicate form as a coversheet*
